AI agent works fine 95% of the time. the problem is the other 5%.

our AI agent handles a few internal ops tasks and occasionally goes off track or just fails halfway through. yesterday it called the right tool, got a valid response, then somehow decided the task had failed and retried the whole thing. logs aren't helping much since nothing is technically "broken"…
